Options for Long Term Care
Jeffco Options for Long Term Care is a Single Entry Point Agency providing Medicaid sponsored services for elderly or handicapped adults who can be appropriately cared for in their own home, rather than a nursing home.
We provide information and referrals on long-term care for Jefferson County residents. We offer everything from resource material and home care advice to nursing home information. We serve the function of being Jefferson County citizens' access to Medicaid long term care by providing information, assessment, care planning, and ongoing long-term case management. Our case-managers also have the responsibility of monitoring long term home health services.
- We provide information by telephone or mail to clients who call or come by in person.
- We make referrals to other agencies that might be helpful for the client.
- We assist clients to apply for Medicaid if needed.
- We assess clients seeking long term Medicaid assistance for home care or nursing home placement. For those approved, we direct them to the appropriate programs.
- We approve or deny the functional eligibility for those we assess.
- We must defend any denials before an administrative law judge should the client appeal our decision.
- We provide ongoing case management for more than 1350 clients receiving home and community-based services. This involves care planning, setting up and monitoring of services with outside providers, and six-month home visits with the clients.
- We review and authorize or deny Medicaid skilled home health services.
- We share and coordinate cases with the Community Centered Boards for clients with developmental disabilities.
- We share and coordinate cases with the Adult Protection team.
- We assess person who are being discharged from area hospitals seeking either home based services or nursing home placement under Medicaid.
HOW ARE WE CONTACTED?
We are usually contacted by telephone (303-271-4216) by clients, potential clients, or their families. We also receive referrals from other social workers, doctor's offices, hospitals, apartment complexes, and other agencies. Some referrals are by 'walk-in' and some we receive by FAX (303-271-4207).
