New Liquor Application

A new liquor application takes 3-4 months. We'll only accept a completed application. You need to submit the following forms: State Form DR8404 or State Form DR8403(for 3.2% FMB application only)

School Affidavit
Dance Application if applicable
Individual History Record DR8404-I
Fingerprint Application (Take to Sheriff's Office)
Sheriff's Clearance Form
Financial Documents
Corporate, LLC, Partnership Documents or Sole Proprietor Eligibility Verification Form
Manager Registration DR8442 if manager is not one of the applicants/owners

You'll also need:

Proof of Property Possession, Deed or Lease/Lease Assignment
* The Lease must be in the name of the Applicant.
8 1/2'' x 11" Diagram of premises - Outline premises in red, (include kitchen in outlined premises) liquor storage in yellow with parking spaces outlined.
Area Map - MapQuest is preferred.
