- State Form DR8404 or State Form DR8404(for 3.2% FMB license only)
- Affidavit of Transfer
- Temporary Permit Application
- Dance Application if applicable
- Individual History Record DR8404-I
- Fingerprint Application (Take to Sheriff's Office when fingerprinted)
- Sheriff's Clearance Form
- Financial Documents
- Corporate/LLC or Partnership Documents
- Manager Registration (If owner/applicant is not the manager)
You will also need:

- Proof of Property Possession: Deed or Lease/Lease Assignment- The Lease must be in the name of the Business Entity
- 81/2'' x 11" Diagram of premises - Outline premises in red, (include kitchen if applicable) liquor storage in yellow with parking spaces outlined.
- Area Map - Map Quest preferred
- List of Liquor Education Classes
