Passport Requirements:

• Completed DS-11 passport application not signed but filled out
• Proof of Citizenship: Certified Birth Certificate, Naturalization Certificate or Previous Passport
• Identification - with picture ID
• 1 Passport Photo in color 2 inches by 2 inches (we now offer passport photos for a $5.00 per person fee)
• Social Security Number (not card) to complete the applicationCredit and Debit cards are not accepted.
• You will need two separate checks or money orders for your application.

Children under the age of 16 need to be  accompanied by both parents.

A young person between the ages of 16 and 18 needs to be
accompanied by at least one parent.

*If you have an emergency situation, traveling in 2 weeks or less,  you may go to the Passport Office in Aurora to get your passport.  You must call first and make an appointment.  The number is 1-877-487-2778.
