Commissioner Lawrence moved that the following Resolution be adopted:

BEFORE THE BOARD OF COUNTY COMMISSIONERS

OF THE COUNTY OF JEFFERSON

STATE OF COLORADO

RESOLUTION NO. CC00-462

RE: HUMAN SERVICES - CONTRACT RENEWAL - COLORADO DEPARTMENT OF HEALTH CARE POLICY & FINANCING FOR THE ADMINISTRATION OF THE SINGLE ENTRY POINT/LONG TERM CARE PROGRAM

Resolved, that the Board of County Commissioners hereby approves and authorizes the Chairman to sign the 2000-2001 Single Entry Point Agency Contract between Jefferson County and the State of Colorado Department of Health Care Policy and Financing. The amount of the contract is $910,575.

Commissioner Holloway seconded the adoption of the foregoing Resolution. The roll having been called, the vote was as follows:

Commissioner Patricia B. Holloway Aye
Commissioner Richard M. Sheehan, Chairman Aye

The Resolution was adopted by unanimous vote of the Board of County Commissioners of the County of Jefferson, State of Colorado.
