Board Member Lawrence moved that the following Resolution be adopted:

BEFORE THE BOARD OF SOCIAL SERVICES

OF THE COUNTY OF JEFFERSON

STATE OF COLORADO

RESOLUTION NO. SS00-05

RE: SOCIAL SERVICES - 2000 COMMUNITY SERVICES BL0CK GRANT FUNDS PROJECT PLAN AND FORM CONTRACT

Resolved, that the Board of Social Services hereby approves and authorizes the Chairman to sign the 2000 Community Services Block Grant (CSBG) Project Plan for submission to the State of Colorado for CSBG program funds.

Resolved, that the Board of Social Services approves the model agreement between Jefferson County and CSBG grant recipients for performance of 2000 CSBG activities, and authorizes the Chairman of the Board to sign agreements with CSBG grant recipients upon approval as to form of the contract by the County Attorney's office.

Board Member Holloway seconded the adoption of the foregoing Resolution. The roll having been called, the vote was as follows:

The Resolution was adopted by unanimous vote of the Board of Social Services County of Jefferson, State of Colorado.
