OF THE COUNTY OF JEFFERSON

STATE OF COLORADO

RESOLUTION NO. SS02-21A

RE: Human Services – Contract – Family Tree, Inc. for 2002

Resolved, that the Board of Social Services hereby approves the Purchase of Service Agreement between Jefferson County and Family Tree, Inc. for 2002 in the amount not to exceed $492,682.00 and authorizes the Chairman to sign the same.

Board Member Lawrence seconded the adoption of the foregoing Resolution. The roll having been called, the vote was as follows:

The Resolution was adopted by majority vote of the Board of Social Services County of Jefferson, State of Colorado.
