How to Report Child Abuse/Neglect Concerns

HOW
Before you contact us by phone, email or by our online form please provide as much of the following information as possible in regard to the child(ren) you are concerned about.  It is important for you to provide as much information as possible to ensure the safety and welfare of the individuals involved.


FIRST, WE NEED:

  • The child's full name
  • age
  • date of birth
  • ethnicity or race
  • parents full names
  • family's valid home address
  • family's phone number
  • specific concerns of abuse/neglect
