After it's been determined that you qualify for the Jefferson County Head Start program, an appointment will be scheduled for you to fill out the necessary paper work. The appointment will take approximately 1 1/2 to 2 hours.

You must bring the following documents to your appointment:

Your child's birth certificate

Your child's current and complete immunization record

A copy of your child's physical examination (exam must have been completed within the last calendar year), (Physical Examination form)

A copy of your child's dental health evaluation (Dental Health Evaluation Form, Evaluacion De Salud Dental)

Proof of your income (W2's, income tax returns, or twelve months of check stubs)

In order for us to request your child's physical or dental examination forms from your doctor or dentist, please fill out our Consent to Exchange Information form and bring it with you to your ennrollment appointment.
