Jefferson County Vital Records
Vital Records issues birth and death certificates, and burial permits, as mandated by Colorado State law. Additionally, the office compiles data pertinent to birth and death statistics for submission to the Colorado Department of Public Health and Environment.
Jefferson County Public Health Vital Records is now able to issue birth certificates for any birth occurring in the State of Colorado from 1903 to present. These are official state issued certificates.If you need a marriage certificate, please call the office of the Clerk and Recorder at 303-271-8186.
Certificates are available at the Vital Records Office Monday through Friday, 8:30 a.m. - 4:00 p.m.

Birth Certificates
Birth certificate = $17.75. Each additional certificate of the same record ordered at the same time = $10.00. MasterCard, Visa, checks or cash.

Death certificate = $20.00. Each additional certificate of the same record ordered at the same time = $13.00. MasterCard, Visa, checks or cash.

Receive birth or death certificates by mail/fax or wait at our office. A valid driver's license is required. If you do not have a valid driver's license, valid ID will be required. For a list of Approved Identity Verification documents, click here.
