Expedited Permanency Planning: 303-239-7033 Home visits for children removed from parental custody. Nursing visit services are provided to children from birth to six years of age, and their siblings, who have been removed from the custody of their birth parents by Jefferson County Department of Human Services. Community Health Nurses provide services to the foster families and birth families. These services include assessment, teaching, counseling, and referral in the areas of growth and development, parenting, nutrition, and health.
