Prenatal, Postpartum, Newborn Home Visit Program
The Prenatal, Postpartum, Newborn Home Visit Program provides a home visit by a Public Health Nurse to women who have recently given birth. Home visits seek to promote healthy mother/infant interaction, prevent illness and/or injury, promote breastfeeding and assure access to community resources and follow up medical care.
Within three to five days of receiving a referral from the hospital, a Public Health Nurse will complete a home visit. This early visit allows the nurse to assess and provide support as appropriate with feeding, bonding, parenting skills and family support. A physical exam is done on the mother and the baby and education is provided on a variety of topics such as immunizations, child care and growth and development. Other important aspects of the Public Health Nurse role is to encourage a new mother to choose a birth control method, to keep her six week postpartum check up and to make an appointment with a primary care provider for her baby.
Please call 303-239-7003 for an appointment.
