NFP Partners for Healthy Families: 303-239-7074

Nurse Family Partnership (NFP) Partners for Healthy Families is a free prenatal and early childhood nurse home visitation program for first time mothers who meet financial guidelines.

Community Health Nurse visit as early in pregnancy as possible and continue through the first two years of the child's life. The nurse works as a partner to support the family in reaching their goals by providing referrals, education and nursing interventions. The program is offered at no cost to the family.

Goals:

• Improve pregnancy outcomes by helping women adopt healthy behaviors.
• Improve child health and development by helping parents provide responsible and competent care for their children.
• Improve economic self-sufficiency by helping parents develop a vision for their own future, plan future pregnancies, continue their education and find employment.

JEFFCO F.I.T. - A Coalition for Families with Infants and Toddlers
Partners for Healthy Families is a member of JEFFCO F.I.T. and just one of several programs dedicated to providing services to families with infants and toddlers.

National Center for Children, Families and Communities
Partners for Healthy Families is a nurse family partnership program developed by The National Center for Children, Families and Communities. This interdisciplinary Center based at the University of Colorado Health Sciences Center.
