The Health Care Access Program assists people with access to health care.

JCPH's Health Care Access Program:
- Assistance with Medicaid application process and documentation
- Pregnancy tests and referrals to prenatal providers
- A visit with a nurse will assist in identifying risk factors and provide immediate counseling.
- Referrals to appropriate JCPH programs

The Healthy Communities Program provides application assistance to uninsured pregnant women and families that may be eligible for Medicaid and/or CHP+. The program also assists those already receiving Medicaid and/or CHP+ in understanding their benefits and accessing medical, dental, vision, mental health and specialty services. We encourage parents to take advantage of the preventive health care benefits for their children and help them to find providers for those services. By using preventive services, children will be less likely to end up in the emergency room for a condition that could have been treated in a doctor's office, or for treatment of a condition that could have been prevented.
Healthy Communities:
Healthy Communities combines EPSDT Outreach and Administrative Case Management Program and the CHP+ outreach into one model to better met the needs of our customers. Healthy Communities focuses on the "life cycle of a client" including all of the activities that must occur for the client to obtain coverage and access to health care services.
Healthy Communities formerly called EPSDT provides children under the age of 21 and on Medicaid with regular health checkups, immunizations and treatment for health problems. It also assists pregnant women with accessing prenatal care.
Who Benefits from Healthy Communities?
Any child or pregnant woman who already has a Medicaid card can get Health Communities' services. If a child does not have a Medicaid card, apply at your local Social Services office or Presumptive Eligibility site.
Family Health Coordinators
The Healthy Communities Family Health Coordinator is assigned to you based on your zip code and can help keep you up to date with your child's health care. This case manager can help you find providers for well or sick care, dental needs, vision checks and eye glasses, and prenatal care for pregnant women.

CHP+: 303-232-6301
CHP+ is a state insurance plan for children through 18 years of age. It is based on income eligibility. There is a small annual fee. Co-pays are based on factors such as income and family size. The Health Department does the pre-qualifying screening for pregnant women only and the final determination is done by CHP+. If you are seeking CHP+ only for children please call the CHP+ program at (303) 751-9051. Jefferson County Human Services can also assist with getting children enrolled in CHP+.

Presumptive Eligibility for Pregnant Women
JCPH coordinators screen pregnant women for eligibility for Medicaid / CHP+. This is a program that gives immediate temporary medical coverage for pregnant women that have applied for Medicaid or CHP+ and pregnant women with children under age 19 who are living in the same household. Temporary coverage provides coverage for no less then 45 days while the client waits for eligibility confirmation. For more information call 303-232-6301.
