Child and Adolescent Health Services

Jefferson County Public Health promotes the health of children and adolescents through many of its program. The department does not offer sick care. Child & Adolescent Health Clinic - immunizations, counseling and referral, physical examinations (adolescents only). Healthy Communities (formerly called Early Periodic Screening, Diagnosis and Treatment (EPSDT) provides administrative case management and outreach to pregnant women and families with children on Medicaid or eligible for Medicaid. Growth and Parenting Program - Multi-disciplinary team case management of children at risk for non-organic failure-to-thrive. Health Care Program for Children with Special Needs (HCP) provides nurse case management for children with special needs. Home visits and Nurse Case Management for pregnant women and their children. Health Care Access - assistance with accessing community health care services and obtaining health insurance. Family Planning - comprehensive reproductive health and birth control services to males and females. Additional Adolescent Health Services HIV (Human Immunodeficiency Virus) counseling, testing and referral. STD (Sexually Transmitted Disease) screening, diagnosis and treatment. Teen SHOP (Sexual Health Outreach Program) provides youth with education and information on health issues and accessing health services.
