The Child and Youth Leadership Commission (CYLC) is the 1451 Collaborative Management Program (CMP) in Jefferson County and is a partnership of 22 family-serving agencies and local jurisdictions with set policies, procedures, and bylaws.
The mission of CYLC is to increase the health and well being of children, youth, and families by establishing collaborative leadership, accountability, and efficient and cost effective coordinated systems, through streamlining services and reducing duplication. CYLC gathers each month and meetings are open to the public.
In 2012, The Colorado Department of Human Services (CDHS) was awarded a System of Care Implementation Grant from the U.S Department of Health and Human Services, Substance Abuse and Mental Health Services Administration (SAMHSA). Colorado is one of 16 sites nationally to be awarded the grant. Key features of Colorado’s system of care framework include individualized and culturally and linguistically relevant services and supports facilitated by an intensive care coordination model known as high fidelity wraparound. Jefferson County was added to the SAMHSA-supported system of care implementation project beginning in 2013. A Family Advocate, System Navigator, and Wrap Facilitator have been added to work with youth and families who meet criteria.
