  • SOAR

SOAR (SSI/ SSDI, Outreach, Access and Recovery) is a service available to severely disabled, homeless or at-risk of homelessness individuals. In Jefferson County, individuals in the Colorado Works program who are applying for Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI) may utilize the services of a SOAR case manager.

A case manager will assist you with applying to speed up the application process.

    • When an individual applies with Social Security on their own, the award rate is close to 15 percent.
    • When an individual applies with Social Security using the SOAR model, the award rate jumps to close to 70 percent!
    • Individuals not in the Jefferson County Human Services Colorado Works program, but who think they may be eligible for Social Security benefits, may access a SOAR case manager directly through Disability Benefits Services, a 501(c)(3) non-profit organization who can assess a client’s case directly. Call 888-396-9838 for assistance.

To be eligible for the SOAR program, the applicant:

    • Must have a documented severe physical or mental disability that prevents them from working 12 months or more.
    • Must have very little or no income.
    • Must be homeless or at-risk of homelessness.
      • This includes living in shelters or motels; living with family or friends; being close to eviction or foreclosure; “couch surfing”; and exiting jail, prison or hospital without a place to live.
    • Must be your first Social Security application or has been denied and wants to apply again (not in appeals).

There is no age or geographical limit.
