Children must be three years-of-age, and not yet five years-of-age, by October 1 of the calendar year in order to be enrolled. Families are encouraged to apply prior to child’s 3rd birthday and can be enrolled immediately upon child turning 3 years old. Families must reside in our service area, which is Jefferson County, excluding the city of Lakewood. Verification of residency is required. (City of Lakewood residents can call 303‐987‐2490 to apply for Lakewood Head Start.)Families must provide verification of income in order to qualify for the program. Regardless of income, foster children, families experiencing homelessness, TANF recipients, and SSI recipients are also eligible for the program. Although the majority of our families are below the federal poverty guideline, we have over-income spaces set aside to serve children with special needs and other circumstances. We accept applications and additional documentation at either of our 2 locations, online, via fax, mail, or via the PEAK website. At the time you submit the application and supporting documents, an intake appointment will be completed with you to determine your child’s eligibility for the program.Please bring the following documents at the time of application:• Completed application• Your child's birth certificate• Verification of residency (Utility bill, lease, mortgage statement, SSI verification, Food Stamp or TANF verification with parent/guardian name and address) • Your child's current and complete immunization record • Proof of your income (W2's, income tax returns, or twelve months of check stubs) Learn more about immunizations on the Jefferson County Public Health website.We will also need the following as soon as possible:• A copy of your child's physical examination ; exam must have been completed within the last calendar year.• A copy of your child's dental health evaluation In order for us to request your child's physical or dental examination forms from your doctor or dentist, please fill out our Consent to Exchange Information form and bring it with you to your enrollment appointment.
